The puppy/dog has received the necessary age appropriate vaccinations and de-wormings. The puppy/dog has been examined by our licensed Veterinarian prior to adoption which includes a fecal exam and full body exam (ears, eyes, mouth, heart etc...) The following conditions apply to the sale of the puppy/dog described above: 1. CONGENITAL GENETIC HEALTH WARRANTY A warranty against congenital defects causing death of or making necessary euthanasia of puppy/dog is provided for up to 2 years from date of birth of puppy/dog. Should the puppy/dog be found to have a congenital defect within the first 2 years of life, Breeder must be notified during this time period. Buyer(s) must establish proof of congenital defect by providing Breeder with all necessary documents, health records and x-rays from Veterinarian(s) and Specialist(s) Buyer(s) Initials_____/_____ 2. GENETIC HIP DYSPLASIA WARRANTY A warranty for (HD) Hip Dysplasia causing poor quality of life or requiring corrective surgery is extended for up to 26 months from birthdate of puppy/dog. Should the puppy/dog be found to have a Hereditatry HD within the first 26 months of life Breeder must be notified during this time period. Buyer(s) must establish proof of HD by providing Breeder with all necessary documents, health records and x-rays from Veterinarian(s) and Pennhip specialis or OFA reading. Health warranty will be terminated should the puppy/dog be: A.) Allowed to become "obese" at anytime during the warranty period. Please note that the joints, tendons and muscle structure will not be fully mature on your puppy/dog until around the age of 2 yrs. therefore, your puppy/dog should avoid high amounts of exercise, hiking, climbing stairs or running for long distances until his or her joints and joint structure are fully developed. B.) PennHip X-ray indicates that an injury may have occurred that caused or contributed to the issue Buyer(s) Initials_____/_____ **Please note that we strongly recommend giving the NuVet vitamin supplements for the lifetime of your dog. This vitamin suppliment not only helps develop strong joints for growing puppies but also boost the immune system to help fight disease for dogs of any age.Your puppy/dog will be sent home with a sample of the NuVet vitamin supplement along with a NuVet pamphlet. The pamphlet will cover detailed information about the NuVet vitamin suppliment as well as information about other NuVet products that are benificial for the health of your puppy/dog. PUPPY REPLACEMENT/REFUND POLICY If your puppy/dog is proven to have congenital defects within the first 2 years or genetic HD within the first 26 months of it's life, Breeder will offer a replacement puppy (of equal value) from next available litter. If Buyer(s) do not want a replacement puppy then Breeder will offer to sell a replacement puppy (of equal value) and reimburse Buyer(s) with a cash refund up to amount of original puppy/dog purchase price. We are a very small scale hobby breeder so please note that it can take up to 1 yr. for puppy placement or cash refund to be given. Buyer(s) will allow breeder and Breeders Veterinarian to communicate with Buyer(s) Veterinarian(s). Failure to comply with this policy and provide all documentation to Breeder will void all warranties on this contract. Buyer(s) Initials_____/____ 4 (a). PET ONLY POLICY Every My Doodle Darlins puppy is to be purchased as a Family pet or working dog only and is not intended to be bred for profit. Buyer(s) agree to spay or neuter this puppy/dog by 6 months of age by a licensed Veterinarian with the exception of written consent from the Breeder. Buyer(s) must provide Breeder with a hand written statement from the performing Veterinarian on the Veterinarian Letterhead stating that the procedure has been completed on or before 6 months of age. If the puppy/dog is not been spayed or neutered within the time period, warranty is void. Buyer(s) Initials_____/_____ 4 (b). BREEDER POLICY This dog is being purchased for breeding and breeder has released registration papers to the Buyer(s). Breeder does not guarantee the reproductive organs of the puppy/dog nor the ability to reproduce. Breeder guarantees genetic health of this puppy for up to 26 mos. Breeder guarantees final OFA, BVA or PennHip scores to pass with a grade of Fair/Good/Excellent/Normal or Breeder will provide Buyer(s) with another puppy/dog of equal value. Proof must be established by providing Breeder with all utd vet health records for the dog as well as any genetic health screening records issued by a qualified specialist before another puppy/dog is released to Buyer(s). Dog must not be bred prior to final health testing at the age of 24 mos or all Warranties will be null and void. Breeders Signature____________________________ Buyer(s) Initials_____/______ 5. MAINTENANCE POLICY A.) Buyer(s) agree to secure the best of Veterinarian care and attend to the physical and emotional welfare of this puppy/dog throughout his/her lifetime B.) Provide Veterinarian care at periodic intervals including regular checkups, worming, heart worm prevention and keeping vaccinations current. C.) Provide a vet recommended high quality food and fresh water such that your puppy/dog will be well nourished and more resistant to sickness. D.) Provide suitable shelter/housing typical for its breed and be adequately protected from the elements and extreme weather conditions if it is to be kept outside. Please note that if the breeder or a Veterinarian were to ever suspect that the puppy/dog is being or has been abused physically or mentally, Breeder has the right to request the dog be returned to them. All Veterinary records must be kept current at all times and provided to Breeder if Buyer(s) are requesting a replacement puppy or refund. Buyer(s) Initials____/_____ 6. LIFE TIME SUPPORT AND RE HOME POLICY My Doodle Darlins will be available to you for the lifetime of your puppy/dog and you may contact us at anytime with any questions or concerns you may have. Buyer(s) agree that if for any reason during the puppy/dogs life, they are unable to keep or provide for their puppy/dog, they will contact the Breeder to take the puppy/dog back or seek our assistance to help in re homing the puppy/dog. Warranties are non transferable and all shipping or delivery cost are the responsibility of the Buyer(s). Under no circumstances shall the Buyer(s) place the puppy/dog in a Shelter, Humane Society, Dog Pound, Pet Mill or Pet Store. Buyer(s) Initials_____/_____ 7. FINAL AGREEMENT In order to validate this contract, the contract must be signed and returned to Breeder prior to puppy/dog pick up or delivery. The Buyer(s) Initials and signature below, indicates you have read, understand and agree to comply with policies set forth in the contract. Buyer(s) understand if any part of this contract is breached the Breeder is released from any obligations under this contract. Buyer(s) also understand that under no circumstances shall the Breeder be liable to the Buyer(s) or any third party for any consequential, incidental or special damages resulting from or in manner related to the puppy/dog. Upon execution of this contract all Veterinary care and its associated cost are the sole and exclusive responsibility of the Buyer(s). This contract shall be construed in accordance with the laws of the state of Arkansas jurisdiction and venue shall lie in a court of competent jurisdiction location in Conway County, Arkansas.
